A) Core Application Requirements (Mandatory for Everyone)
- DS-160 Form (Online Application)
- Fill at ceac.state.gov.
- Upload a compliant U.S. visa photo.
- Print DS-160 confirmation page with barcode.
- Visa Appointment (USTravelDocs)
- Create profile at ustraveldocs.com/th.
- Pay MRV visa fee (~USD 185, in Thai Baht).
- Book appointment at U.S. Embassy Bangkok or U.S. Consulate Chiang Mai.
- Print appointment confirmation page.
- Passport (Original + Copies)
- Valid at least 6 months beyond intended stay in the U.S.
- At least two blank pages.
- Include old passports with prior visas/travel.
- Photograph
- Size: 2×2 inches (51×51 mm), color, white background, no glasses.
- Upload electronically and bring 1 printed copy.
- Visa Fee Receipt
- Proof of MRV payment.
B) Medical-Specific Supporting Documents
1. Medical Diagnosis in Thailand
- Letter from treating physician or hospital in Thailand stating:
- Nature of the medical condition.
- Why treatment is required.
- Why the treatment cannot be fully provided in Thailand.
- Recommendation to seek treatment in the United States.
2. U.S. Hospital/Doctor Documentation
- Letter from U.S. hospital or doctor confirming:
- Willingness to treat the applicant.
- Details of proposed treatment plan.
- Estimated duration of treatment.
- Estimated cost of treatment (consultation, hospital stay, procedure, etc.).
3. Proof of Ability to Pay for Treatment & Travel
- Financial evidence showing coverage of medical expenses + living costs:
- Bank statements (3–6 months).
- Savings/fixed deposits.
- Insurance documents (if international coverage).
- Proof of sponsor’s finances (if a family member or organization will pay).
- Affidavit of Support (Form I-134) (optional but helpful if sponsored by a U.S. family member).
4. Arrangements in the U.S.
- Evidence of accommodation during treatment (hotel, hospital lodging, or staying with relatives).
- If staying with family/friends: letter of support + copy of their ID/Green Card/passport.
C) Additional Supporting Documents (Standard B1)
- Employment documents (if applicable):
- Employment verification letter (position, salary, leave approval).
- Recent payslips.
- Business ownership (if self-employed):
- Business registration certificate, tax filings.
- Family/social ties in Thailand:
- Marriage certificate, birth certificates of children.
- House registration (Tabien Baan).
- Property deeds or land ownership.
- Vehicle registration.
- Investments (stocks, funds, bonds).
- Previous travel history:
- Copies of visas (U.S., Schengen, UK, Japan, Korea).
- Stamps showing compliance with return.
D) Embassy Interview Preparation
- Officers will focus on:
- Medical necessity: Is treatment required in the U.S.?
- Financial ability: Can you afford the treatment and living expenses?
- Ties to Thailand: Will you return after treatment?
- Common questions:
- What is your medical condition?
- Why do you need treatment in the U.S.?
- Which doctor/hospital will treat you?
- How much will the treatment cost?
- Who will pay for your treatment and trip?
- What ties do you have to Thailand?
E) Pre-Interview Checklist
